Specialty Post Acute Care (S-PAC) is a specialized, nationwide managed healthcare network that supports the acceleration of patient transfers from acute inpatient hospitals into alternative, more cost effective post acute care facilities. The S-PAC network is comprised primarily of skilled nursing / subacute care, acute inpatient rehabilitation and long term acute care facilities at guaranteed, comprehensive, per diem rates. S-PAC is designed to assist health insurance payors in reducing the cost of extended inpatient episodes by promoting the effective use of post acute care services. S-PAC's clients have experienced lower costs due to appropriate placement of patients in lower level of care facilities, per diem rates for post acute care services, reductions in acute care length of stays, and better overall outcomes.
S-PAC differs markedly from conventional Preferred Provider Organizations by providing guaranteed per diem rates, assistance with placement of the patient, timely information and notices, and nationwide access to credentialed facilities.
